BIRTHDAY PARTY RELEASE
By signing this release, I give permission to my child and/or ward___________ age_____ to participate in Rhythmic Dreams activities. I acknowledge rhythmic gymnastics is an active sport which requires strength, agility, and concentration and involves twisting, turning, jumping, flexing, extension, and rotation, movements which can result in severe, permanent personal injuries, included but not limited to bruising, strained, sprained, or torn muscles, tendons, or ligaments, broken bones, derangement or dislocation of joints, paralysis or death. In case of emergency I give Rhythmic Dreams my permission to use its discretion in determining whether my child and/or ward requires medical attention and if so, to use discretion when transporting my child selecting a health care facility and obtaining treatment for him or her. I release Rhythmic Dreams, inc. and its employees from any and all liability for personal injury to me and to my child and/or ward before during or after my child's participation in Rhythmic Dreams activities. On behalf of myself and my child I AGREE to indemnify and hold harmless Rhythmic Dreams, inc. and employees from any and all claims, demands, costs, expenses and compensation arising out of the course or in any way related to any personal injury to me or my child.
I give permission to Rhythmic Dreams staff to make and display photographs of my child within their facilities, printed materials and/or their websites.
I acknowledge that I read, understood, and agreed with the above policies and the liability waiver.
PHOTO and VIDEO RELEASE
By participating in Rhythmic Dreams Inc. or Boston Rhythmic Inc. classes, events and programs, I understand that my child's photographs and videos might be used in Rhythmic Dreams or Boston Rhythmic publications, both individual or in groups, both in print or in electronic media (such as Rhythmic Dreams website, brochures, YouTube, Facebook, etc.). I give permission to Rhythmic Dreams Inc and Boston Rhythmic Inc to use my child's photographs in their print and electronic media. In case I do not agree with my child's photographs/videos to be used by Rhythmic Dreams/Boston Rhythmic, I have to submit a written request to the office.
I have read the above and have been given an opportunity to speak with a representative of Rhythmic Dreams and Boston Rhythmic before signing this Medical and Liability Release, Payment Policy and Photo and Video Release. I agree and commit to follow these policies for the duration of my child's enrollment in Rhythmic Dreams and Boston Rhythmic programs.

MEDICAL and LIABILITY RELEASE
By signing this release, I acknowledge my understanding and acceptance of the following:
· That rhythmic gymnastics can result in injury to participants and/or spectators
· That it is solely my responsibility to consult with my child's physician and to determine that my child is in good health and good physical and mental condition before permitting my child to exercise, work out, receive instruction and perform. Starting with June 20 2016, ALL STUDENTS ARE REQUIRED TO HAVE A CURRENT VACCINATION AND MEDICAL RELEASE NOTE FROM THEIR DOCTOR in file, not older than 12 months.
· That rhythmic gymnastics is an active sport which requires strength, agility and concentration and involves twisting, turning, jumping, flexion, extension and rotation, movements which are performed with considerable force and/or at considerable height and which can result in severe, permanent personal injuries, including but not limited to bruising, strained, sprained, or torn muscles, tendons or ligaments, broken bones, derangement or dislocation of joints, paralysis, or death.
· That Rhythmic Dreams, Inc. and Boston Rhythmic Inc. provide an area for observing my child and/or ward during his or her activities and that I have the option to remain in the observation area while my child is in a class, working and performing.
· That in the event I choose to leave my child before or during class, work out or performances, I have given Rhythmic Dreams and Boston Rhythmic my permission to use its discretion in determining whether my child and/or ward requires medical attention in case of an emergency and if so, to use discretion in transporting my child, selecting a health care facility and obtaining treatment for him or her.
I hereby release Rhythmic Dreams, INC. and Boston Rhythmic Inc. its owners, instructors employees, agents and servants, from any and all liability for personal injury to me and to my child and/or ward before, during and after my child's participation in Rhythmic Dreams Inc. and Boston Rhythmic Inc. classes, camps, events or other programs whether at a Rhythmic Dreams, Inc. and Boston Rhythmic Inc. facilities or elsewhere. On behalf of myself and my child I AGREE to indemnify and hold harmless Rhythmic Dreams, Inc. and Boston Rhythmic Inc. and their owners, operators, instructors, employees, agents and servants from any and all claims, demands, costs expense and compensation arising out of the course of in any way related to any personal injury to me or to my child.
To register for a class, please call us to discuss proper level and placement for your daughter or to arrange a trial class.
At the registration page, you will be asked to enter basic information for you and the prospective student, along with an email/password combination that you will be able to use to log in in the future.
Rhythmic Dreams is the sole owner of the information collected on this site. We only have access to/collect information that you voluntarily give us via email or other direct contact from you. We will not sell or rent this information to anyone. We will not share your information with any third party outside of our organization, other than as necessary to fulfill your request, [e.g. to ship an order of gymnastics equipment, register a child for competitions, etc].
We use email to communicate with our clients about program details and schedules and share promotions, new products or services. Therefore, one valid email address is required in each student's records as long as the student takes classes with us. At all times, via our online records database, you can see what data we have about you, change, correct or delete any such data.
Once you stopped taking classes, you may opt out of any future contacts from us at any time by contacting us via email address or phone number (email@example.com or 617-233-4493).
School Calendar: Rhythmic Dreams and Boston Rhythmic follow an academic yearly calendar starting on the Tuesday after Labor Day and ending between June 15 and June 30, depending on the number of snow days observed. An exact calendar will be posted online each year by July 15 and updated on April 15. We are also closed for classes (and offer camps at extra cost) during all school vacations (December, February, April and Summer) and we are open on most smaller national or religious holidays.
Annual Tuition: Tuition is based on a two-semester school year, with flat amounts for each semester. First semester tuition (September through January) is due on July 1st. Second semester tuition (February through June) is due on January 1st.
Monthly plans: In case of financial difficulty, a monthly payment plan (automatic credit card withdrawal) can be set up with the approval of the office. Tuition will be divided in NINE installments, with the following dates: Aug 1st, Sep 1, Oct 1, Nov 1, Dec 1, Jan 1, Feb 1, Mar 1, Apr 1. An additional 3% credit card fee will be assessed for the monthly payment plan. NO WITHDRAWAL request will be approved after April 1st.
Registration fee: At the time of registration, each student should submit the following fees a registration fee of $110 for NEW students, $90 for continuing students (good for the school year), which covers one training outfit (leotard, shoes and bag) and all costumes for shows and exhibitions.
All other fees are due immediately after invoice - competition, equipment, private classes, etc.
All late payments will incur a $10 late fee.
If your check is returned for insufficient funds, you must submit a new check, payable to Rhythmic Dreams, for the original amount plus a $25 returned check fee, within ten (10) business days, in order to avoid additional late fees.
The lower tuition sibling will receive a 10% discount on the class tuition fees ONLY. There are no discounts on camp fees, registration fees, competition fees, merchandise purchases or any other fees.
There are no refunds for any missed classes (vacations, sick days). Please schedule a make-up within two weeks. Three make-up classes are allowed per semester.
Summer camp tuition is due at full at registration time. Full refund for summer camp is available before April 15. No refunds after April 15. Week transfer fee after April 15 is 25$.
Full refund of unused tuition will be issued if Rhythmic Dreams/Boston Rhythmic is given a one month notice from the due date of the payment. (For example, if January is the last month of training, we need notice by December 1st--which is the due date for the January tuition--and the last day of training will be January 31st. You will receive a full refund for all tuition paid past February 1st).
